Pre-evaluation form for new patients

Thank you for your interest in scheduling your visit with Bioadvanced Medical Center. To provide you with the best care, we kindly ask for your help in filling out this pre-evaluation form with your contact information and a detailed, complete description of your medical situation.

This pre-evaluation form will help us to:

  • Obtain more information about your medical case and determine if you qualify for our treatments.

  • Determine the number of days recommended for your first visit.

  • Identify any contraindications with current treatments.

  • Verify if laboratory/imaging studies are needed prior to your visit.

Once this information is received, it will be reviewed by our medical team, who will contact you if additional information is required. If no further information is necessary, you will receive an email from our Scheduling department to continue with your appointment request.

Please allow 5 to 7 days for a response due to the high volume of requests we receive daily.

Before filling out your form, please consider the following:

  • We cannot confirm costs or specific treatments to be performed until you have your in-person consultation with the doctor, as it is necessary to gather additional information about your medical condition, such as lab results, imaging studies, medical history, etc.

  • It is essential to have the travel documents required by your country of origin (if you are outside of Mexico) before starting the scheduling process.

  • Please submit only one form per patient; submitting more than one may result in your information being deleted due to duplication.

  • We highly recommend not making travel plans until your request has been evaluated and approved by our Medical team.

  • You will receive an automatic email to confirm that your pre-evaluation form has been submitted (please check your Spam folder if you do not receive it in your inbox).

Traveling to Bioadvanced Medical Center

Let us provide you with some tips to make your travel experience the most enjoyable one

Ensure passports are valid for the duration of your stay and packed in an accessible place. Keep a copy of important documents (i.e. passports, personal identification, flight itinerary, etc.) with someone you can contact in the event of loss/theft. While these events are uncommon for our patients to experience, preventative measures are important for any international traveler.

Most patients traveling by air to Baja arrive at San Diego International Airport (SAN). To confirm your clinic stay, please forward confirmed flight itinerary to the clinic as soon as possible. We’ll be more than happy to help you through any questions you may have.
